Membership Guide:
Names of physician members of the Foundation for Medical Care of Tulare and Kings Counties are included in the Provider Search link.
As a foundation subscriber or dependent, you may select the services of any physician you wish. If you select a physician member of the Foundation, payment will be made directly to him/her in accordance with your health services agreement.
In most instances, Foundation payment to member physicians is considered payment in full, subject to copayments and deductibles, for covered benefits. Additions and changes to this membership list are made continuously. Revised lists will be published periodically, however you should check with your doctor to see if he/she is a member of the Foundation for Medical Care network, or if you have any questions, you may call the Foundation and we will assist you. To eliminate unexpected expense, it is good practice to discuss costs with your doctor before treatment, regardless of whether or not your doctor is a Foundation member physician.
How to File a Claim:
To obtain benefits under your Foundation for Medical Care sponsored plan -
- Present your identification card to your physician or hospital. Ask them to bill us directly or you may send us an itemized bill.
- The only info we need to process your claim for payment is:
- Patient Name
- Group Number
- Subscriber ID Number
- Birthdate of Patient
- Itemized Bill
- A Diagnosis
The Foundation may require additional information regarding other medical insurance, date of injury, etc. Please answer and return all such inquiries as soon as possible. If you have other health insurance, the Foundation will coordinate its payment with the other coverage.
